firsthand is seeking a licensed Nurse Practitioner to serve as a Local Health Guide, providing primary and urgent care to individuals with serious mental illness. This part-time role involves in-person care in various settings, focusing on whole-person assessments, chronic care follow-up, and urgent interventions. The Local Health Guide will collaborate closely with an interdisciplinary team to support individuals on their journey to better health.

Practice Info

•Provides primary and urgent care in-person care (home, street, office setting)

•Serves individuals with serious mental illness

•Collaborates closely with specialists psychiatric registered nurses, primary care nurse practitioners, and psychiatric nurse practitioners

•Works as part of an interdisciplinary care team with certified peer recovery specialists (firsthand Guides)

Responsibilities

•Provide in-person whole person assessments, chronic care follow-up, and urgent care

•Assess both behavioral and physical health conditions with a thorough history and in-person physical examination

•Capture conditions not yet diagnosed, ordering additional studies where indicated to confirm or rule out the diagnosis

•Provide primary and urgent-care treatment to minimize preventable ER visits for conditions such as COPD exacerbations, uncontrolled diabetes, CHF exacerbations, Cold and Flu Symptoms, UTI, pain, anxiety, depression, and suicidal ideation

•Collaborate regularly with certified peer recovery specialists (firsthand Guides) on individualized recovery plans

•Make individualized recommendations and/or referrals for programs/care provided by firsthand and/or outside providers

•Support individuals in developing and exercising self management skills and person-centered care around their individual goals and preferences via motivational interviewing and holistic care planning

•Organize clinical information in a manner that supports appropriate clinical documentation and successful disability applications

Compensation

•Hourly Pay Rate: $75-$75 USD

Benefits

•This role is not benefits eligible.

Shift & Schedule

•Part-time role, estimated at working about 20 hours/week

•Work at least three days per week of in-office or home care

Requirements

•1+ years clinical care as a Nurse Practitioner, ideally in a home or community primary care setting, or emergency department

•Able to prescribe medications for behavioral health conditions including antipsychotics

•Working with and managing a population with challenges such as behavioral health and/or SUD conditions

•Care management and coordination

•Proficiency and ease with use of technology for documentation or coordination of care (e.g. electronic health or care management software, direct messaging)
