We are seeking compassionate and driven Nurse Practitioners to join our dynamic Clover Home Care teams in Essex County, NJ. This is a unique opportunity to provide comprehensive primary and supportive care directly in the homes of some of our most vulnerable members, helping to improve health outcomes and quality of life.

As a Clover Home Care Nurse Practitioner, you’ll join our Clover Care Visit (CCV) Team. The CCV team focuses on early intervention and transitional care, helping to set the foundation for long-term health and reduce avoidable complications. You’ll work closely with our clinical insight platform, Clover Assistant, to support decision-making and improve outcomes.

A reasonable estimate of the base salary range for this role is $140,000 to $165,000. Final pay is based on several factors including but not limited to internal equity, market data, and the applicant’s education, work experience, certifications, etc.
