Nurse Practitioner - Hospice - PRN

Called to serve patients in their homes, the Nurse Practitioner approaches each day with a passionate commitment to delivering personalized, one-on-one care — helping patients regain their independence and achieve their health goals. In this role, you will provide hospice care through the evaluation and assessment of the overall medical conditions of patients, complete face to face assessments and evaluate hospice eligibility.
