About the position
The Department of Public Health (DPH) is recruiting for Nurse Practitioners II (NP II) who provide patient care services in primary care clinics with behavioral health integration using the collaborative care model. Duties include obtaining patient health history; performing physical examinations; identifying abnormal conditions and developing treatment plans; recording findings, assessment, treatment plans, follow-up examinations, and subsequent evaluations; engaging mental health screenings and providing interventions with support from a clinical therapist and Psychiatrist. Immediate vacancy exists in the Ontario Federally Qualified Health Center. The list established from this recruitment may be used to fill future vacancies throughout Federally Qualified Health Centers in San Bernardino and High Desert as they occur.
