We are seeking a dedicated and compassionate Psychiatric Mental Health NP) to join our dynamic healthcare team. This role involves providing psychiatric/mental health/medication management services to the residents of post-acute/long-term care facility residents. You will be instrumental in enhancing our residents' quality of life by delivering expert medical assessments, managing complex health conditions, and coordinating multidisciplinary care plans with a focus on their mental health needs. Your expertise will help foster a safe, supportive environment where residents feel valued and well-cared for.
•Conduct thorough medical evaluations and physical examinations of residents to assess their health status and identify psychiatric/mental health needs.
•Develop, implement, and regularly update individualized care plans tailored to each resident’s unique health requirements.
•Manage acute and chronic health conditions, including medication management, treatment adjustments, and necessary interventions.
•Collaborate closely with nursing staff, specialists, therapists, and family members to coordinate comprehensive care delivery.
•Oversee medication administration processes, ensuring adherence to safety protocols and regulatory standards.
•Perform routine follow-ups, monitor vital signs, and document all clinical interactions accurately within electronic health record (EHR) systems.
•Participate in quality improvement initiatives, discharge planning, and clinical research activities to enhance resident outcomes.
Skills
•Knowledge of and willingness to learn medical terminology, physiology, and pathology relevant to geriatric populations.
•Proficiency with electronic health record (EHR) systems.
•Experience with inpatient care settings, hospital medicine, or post-acute care environments is highly desirable.
•Familiarity with specialized fields such as psychiatry, cardiology, endocrinology, nephrology, pulmonology, or neurology is advantageous.
•Ability to perform comprehensive physical examinations and diagnostic evaluations efficiently.
