Pennsylvania Licensed Telehealth PA/NP

A leading healthcare provider is seeking a dedicated Physician Assistant/Psych Nurse Practitioner to deliver high-quality telehealth services for geriatric patients.

Key Responsibilities:

•Conduct thorough telehealth follow-up visits for psychiatric medication management in elderly patients, closely monitoring treatment response and adjusting medications as necessary due to age-related risks and medical comorbidities.

•Perform comprehensive cognitive screenings and develop initial care coordination and behavioral support plans tailored to individual patient needs.

Qualifications:

•Licensed and certified as a Physician Assistant or PMHNP in Pennsylvania with current active licensure status required.

•Experience in geriatric psychiatry, long-term care, or dementia care highly preferred; strong clinical background essential.

•Familiarity with various cognitive assessment tools and evidence-based behavioral management strategies essential for effective patient outcomes.
