General

Conduct and review mental health assessments and necessary follow up services. Review Assessments and Treatment Plans conducted by QMHPs at minimum annually or designate this task to qualified staff. Consult with clinic team as needed. Provide diagnostic evaluations as needed, medication prescribing and follow-up. Work with OPMH clinic team and Behavioral Health Residential team to provide care coordination to clients.

Status: Full Time and Part Time available

Location: Salem Clinic and/or Eugene Clinic

Shift Options: 8 or 10 hours (depending on availability and home needs)

Hourly Wage Scale: $55.25 to $70.43

Skill & Effort:

Licensed Psychiatric Mental Health Nurse Practitioner (PMHNP) or

Physician Assistant (PA) in the State of Oregon

Training, experience, and demonstrated competence in:

Current professional licensure;

Required insurance documentation (as applicable)

Applicable federal and state healthcare regulations

Ability to:

Electronic medical records (EMR);

Computer-based clinical documentation systems
