Psychiatric Mental Health Nurse Practitioner (PMHNP)
Deer Oaks Behavioral Health — Long-Term Care Clinical Services
In this role, you will be assigned to a facility or facilities in your local community, allowing you to build meaningful connections with the individuals you serve. You will have the opportunity to conduct in-person psychiatric assessments, medication management, and implementing Medicare compliant gradual dose reduction strategies for psychotropic medications. Clinical Care
•Conduct comprehensive psychiatric evaluations and mental health assessments for LTC residents
•Develop, implement, and adjust individualized treatment plans
•Prescribe and manage psychotropic medications in accordance with Medicare and LTC best practices
•Implement and document Medicare-compliant gradual dose reduction (GDR) strategies
•Monitor and treat chronic mental health conditions including depression, anxiety, bipolar disorder, schizophrenia, and dementia-related behavioral disturbances

Collaborate with interdisciplinary teams—nursing, medical providers, social services, rehabilitation, and facility leadership
•Complete accurate documentation of assessments, treatment plans, and progress notes using the electronic medical record (EMR) system
Interdisciplinary Collaboration
•Work closely with nursing, social services, rehabilitation, and medical providers to ensure coordinated psychiatric care.
•Participate in care plan discussions, facility meetings, and case collaborations as needed.
Documentation & Compliance
•Maintain complete and timely documentation using electronic medical records (EMR).
•Follow ethical, professional, and regulatory guidelines for psychiatric documentation and care delivery.
Facility Partnership
•Build strong, professional relationships with facility staff and leadership.
•Serve as a psychiatric resource and trusted partner for supporting residents’ mental and emotional needs.
