About the position
Our organization is expanding its behavioral health team and is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to support our partnered nursing homes throughout Saten Island . We specialize in delivering high -quality, compassionate mental health services to older adults in long -term care settings. This position is ideal for a clinician who is passionate about geriatric psychiatry and dedicated to supporting the emotional and psychological wellness of aging populations.
Responsibilities
•Conduct detailed psychiatric assessments and follow -up evaluations for elderly residents in nursing home environments.
•Create individualized treatment plans tailored to each patient’s mental health concerns and diagnostic profile.
•Provide ongoing psychiatric medication management and make clinical adjustments as needed.
•Work collaboratively with physicians, psychologists, social workers, and facility staff to ensure a coordinated, whole -person care approach.
•Deliver therapeutic interventions, including counseling and supportive psychotherapy.
•Guide patients and families by explaining diagnoses, treatment paths, and strategies for managing mental health conditions.
•Maintain clear, accurate, and compliant documentation within electronic health record systems.
•Participate in team conferences, interdisciplinary meetings, and quality improvement efforts within the organization.
