Impact Health PC is seeking a Psych Nurse Practitioner to join their team in Monroe, CT. This role offers the opportunity to thrive in a collaborative environment, providing psychiatric services within skilled nursing facilities. The position emphasizes patient care and offers a flexible Monday through Friday schedule with no nights, weekends, or holidays.
•Advanced practitioner group in skilled nursing facilities
•Specializes in Medical, Psychiatric Services, Wound Care and Palliative Care
•Deliver independent care with access to physician-level consults
Responsibilities
•Deliver person-centered care using evidence-based practices
•Manage caseload and fulfill psychiatric consultation needs of skilled nursing facility
•Provide assessment, diagnostic, and treatment services
•Provide psychoeducation and medication education to patients and/or families
•Collaborate with interdisciplinary facility teams
•Complete timely documentation (within 24 hours)
•Ensure compliance with state regulations in SNF settings
Compensation
•Earning potential: $115,000 (salary based on experience + easily achieved monthly bonuses)

Shift & Schedule

Requirements
•Licensed Family, Geriatric, or Psychiatric Nurse Practitioner in the state of practice
•Experience in skilled nursing facilities, hospitals, residential care, geriatric services, and outpatient psychiatric settings preferred
•Minimum of 2 years of clinical experience providing mental health care to adult populations
•Proficient with EHRs and clinical technology
•Strong communicator with a collaborative mindset
