Mid-Columbia Center for Living (MCCFL), a Certified Community Behavioral Health Clinic, is seeking a dedicated Nurse Practitioner to join its Integrated Care Team. This role focuses on providing clinical care and mental health services to clients with mental illness and addictions, utilizing a trauma-informed and recovery-oriented approach. The Nurse Practitioner will contribute to a whole-person care model for adults and children across Hood River, Wasco, and Sherman Counties.

•Care Model: Trauma-informed, recovery-oriented, consumer-involved, consumer-engaged, and consumer-driven
•Patient Population: Persons with mental illness and addictions, including adults and children, receiving whole-person care
•Team: Works within the Medical / Integrated Care Team and Community Support Services programs
Responsibilities
•Integrate primary health care with mental health treatment of clients accessing services in clinical and community settings
•Provide clinical services in accordance with nurse practitioner licensure for the State of Oregon, including assessment and management of medications for ADD/ADHD and Medication Assisted Treatment clients, behavioral health diagnosis, and Psychiatric Medication Management of stable patients
•Perform the duties of the Enhanced Care Services/Enhanced Care Outreach Services Nurse as needed
•Provide services to 11-14 patients per day
•Coordinate medical and social needs of clients to enhance treatment, monitor chronic disease registries, and assist with case management and health education
•Review and evaluate assessments, treatment plans, diagnosis reviews, progress notes & medication delivery in accordance with State standards
•Administer intelligence, psychological and other behavioral tests within professional qualifications
•Provide medical case management and referral services in collaboration with Community Partners
Compensation
•Pay Range: $75,212.83-87,988.38 Annually

Shift & Schedule
•0.6 FTE (22.5 hours/week)
•Three-day workweek, totaling 22.5 hours
•Scheduling will take place between Monday and Friday, within standard business hours of 8:30 a.m. to 5:00 p.m
Requirements
•Professional licensure in the State of Oregon as an Advanced Practice Registered Nurse (Nurse Practitioner) with a specialty in Psychiatric and Mental Health
•Current certification in First Aid and Basic Life Support
•Valid Oregon or Washington Driver's license, maintain an acceptable driving record, and be insurable for client service purposes and for travel
•Preferred: Experience as a Nurse Practitioner
•Preferred: Experience in a psychiatric or community behavioral health setting
•Preferred: Bilingual fluency in Spanish/English
