- 15-minute med checks on repeat
- High patient volume, low clinical depth
- Transactional relationships with minimal follow-through
- Limited autonomy, rigid protocols
- Someone else's vision, someone else's practice
This is different.
We're looking for a psychiatric nurse practitioner who wants to build their own thriving practice within our innovative wellness clinic. You'll own clinical programs (especially our Spravato initiative), develop long-term therapeutic relationships with patients who actually get better, and have the autonomy to practice psychiatry the way you always wanted to.
If you want to do meaningful work with treatment-resistant patients, integrate cutting-edge therapies, and build something significant—keep reading.
You'll be joining Vita Nova at a pivotal growth stage. We're a physician-led ketamine and wellness clinic that's established (200+ patients treated) but still small enough that your impact is immediately visible.
- Build and manage our FDA-approved, insurance-covered esketamine program from the ground up
Vita Nova Ketamine & Wellness Clinic is hiring for this PMHNP position. Psychiatric Mental Health Nurse Practitioners play a critical role in addressing the growing demand for mental health services across the United States. This employer is actively seeking qualified candidates to join their team.

- This is YOUR program—you design protocols, manage patient journeys, and create the standard of care
- Insurance-covered = patients can afford long-term treatment = you build lasting therapeutic relationships
- Target: 15-25 active Spravato patients within 6 months (you'll have full support to get there)
- 30-45 minute appointments (not 15-minute med checks)
- Medication management integrated with supportive therapy
- Patients with treatment-resistant depression, PTSD, anxiety, chronic pain—complex cases that are actually interesting
- Freedom to create individualized treatment plans that actually work
- Ketamine infusion therapy for rapid symptom relief
- Spravato for sustained, insurance-covered treatment
- Traditional psychopharmacology when appropriate
- IV nutritional psychiatry (NAD+, vitamin therapy)
- Holistic, evidence-based approaches tailored to each patient
4. YOU'LL BUILD LONG-TERM THERAPEUTIC RELATIONSHIPS
- Patients stay with you for months or years, not weeks
- You see them get better, not just rotate through
- Outcomes matter—we track PHQ-9, GAD-7, and celebrate improvements with patients
- Continuity of care, not crisis management
- You're the psychiatric expert—you make the clinical decisions
- Collaborative team environment, but you own your patient panel
- Flexibility to use telehealth strategically for patient convenience
- Design your schedule in partnership with clinic needs
- Small team = your voice matters in how we grow
- Contribute to service development, protocol design, patient experience improvements
- Potential to develop new programs as the clinic expands
- Real leadership opportunity, not just being a cog in a machine
- Are entrepreneurial and want to build something (not just execute someone else's protocol)
- Get energized by complex, treatment-resistant cases (not frustrated by them)
- Believe in therapeutic relationships, not transactional med checks
- Are excited about ketamine/Spravato and want to become an expert
- Value clinical outcomes over patient volume
- Want autonomy and are comfortable with accountability
- See opportunity in being part of a small, mission-driven team
- Are growth-minded and open to integrative/holistic approaches
- Prefer high-volume, low-depth practice (we're the opposite)
- Want everything structured and decided for you
- Are skeptical of ketamine therapy or resistant to new modalities
- Need corporate infrastructure and extensive hand-holding
- Just want a paycheck without building anything
- Prefer working in isolation without team collaboration
We're a physician-led ketamine and wellness clinic specializing in treatment-resistant mental health conditions and chronic pain.
- Spravato (FDA-approved esketamine nasal spray, insurance-covered)
- Ketamine Infusion Therapy (IV ketamine for depression, anxiety, PTSD, pain)
- Comprehensive Psychiatric Medication Management
- IV Nutritional Therapy (NAD+, Myers' Cocktail, immune support)
- Integrative Wellness Approaches
- Co-founded by Leslie (CRNA), Dr. Brijen Joshi (MD), and Yvonne Achi (CRNP)
- LegitScript certified (credibility and compliance matter)
- Small, tight-knit team where everyone's contribution is visible
- Mission-driven: we genuinely care about outcomes, not just volume
- Evidence-based but innovation-friendly
- People who've tried antidepressants without success
- PTSD patients failed by traditional therapy
- Chronic pain patients let down by conventional pain management
- High-functioning professionals seeking cutting-edge treatment
- People who need us because nothing else has worked

- Competitive salary based on experience and patient panel size
- Performance bonuses tied to patient outcomes and practice growth
- Potential for partnership/equity as clinic expands (for the right person)
- Start part-time/per diem (2-3 days/week) while building your panel
- Transition to full-time as patient demand grows
- Flexibility around specific days/hours in partnership with clinic needs
- Strategic use of telehealth for efficiency and patient convenience
- Continuing education support

- Marketing and patient acquisition handled
- Insurance verification and prior authorization support for Spravato

- Psychopharmacology expertise across medication classes

Don't have ketamine experience? That's fine—we'll train you. What matters more is your mindset, clinical skills, and excitement about learning.
This isn't just a job—it's a platform to build your career.
- Build thriving patient panel
- Own Spravato program
- Establish yourself as a psychiatric expert in the Baltimore area

- Potential to lead additional clinic locations as we expand
- Develop subspecialty programs (trauma, perinatal, etc.)
- Build your reputation as a leader in innovative psychiatry
We're not just hiring an employee—we're looking for someone who wants to grow with us.
1. RESUME/CV highlighting your psychiatric NP experience
- Why ketamine/Spravato psychiatry interests you
- What "thriving practice" means to you
- Why you're interested in a smaller clinic vs. hospital/corporate setting
STEP 1: Video screen with Leslie (20-30 minutes)
- Discuss your background, practice philosophy, interest in the role
- Answer your questions about clinic, patient population, expectations
STEP 2: In-person interview with clinical team (60-90 minutes)
- Meet Dr. Joshi and Yvonne
- Tour the clinic, see treatment rooms
- Discuss clinical scenarios and treatment approach
- Review compensation and schedule details
STEP 3: Working interview (optional, half-day, paid)

- Get feel for daily workflow and team dynamics
STEP 4: Offer decision within 48 hours

- Onboarding: 1-2 weeks of orientation to ketamine protocols, EHR, team workflows, and Spravato certification
Q: What's the patient population like?
A: Mostly adults 25-65 with treatment-resistant depression, PTSD, anxiety, and chronic pain. Many are high-functioning professionals who've failed multiple traditional treatments. They're motivated, engaged, and genuinely want to get better.
Q: How much autonomy will I really have?
A: You make all clinical decisions for your patients. We collaborate on protocols and complex cases, but you're the psychiatric expert and we trust your judgment. No micromanagement.
Q: What if I don't have Spravato experience?
A: We'll train you. Spravato certification is straightforward, and we'll support you through the learning curve. What matters is your willingness to become an expert in this modality.
Q: Is this really part-time or am I expected to work full-time hours?
A: Start is genuinely part-time (2-3 days/week). As your patient panel grows and demand increases, there's opportunity to transition to full-time. We're flexible and want to match your schedule to patient need.
Q: What does "competitive compensation" actually mean?
A: Base salary range depends on experience but is market-competitive for Baltimore. Performance bonuses are tied to patient panel size, outcomes, and clinic growth. We're open to discussing specifics with serious candidates.
Q: What's the clinic culture like?
A: Small, mission-driven, entrepreneurial. We're serious about clinical excellence but not corporate or bureaucratic. Direct communication, high standards, mutual respect. We support each other and celebrate wins together.
Q: Can I do telehealth?
A: Yes, strategically. Initial evaluations and complex cases are in-person. Stable follow-ups can be telehealth for patient convenience. Spravato is always in-person (FDA requirement).
Mental health care in America is broken. Patients cycle through providers who don't have time to really help them. Medications are prescribed without therapeutic relationships. People give up hope.
We're building something different.
A place where patients get real time, real care, and real results. Where providers have the autonomy to practice medicine the way it should be practiced. Where innovation meets compassion.
If you're reading this and thinking, "This is exactly what I've been looking for," then we should talk.
We're not for everyone. But for the right PMHNP, this is a once-in-a-career opportunity.
