Job Summary:
We are seeking a highly skilled Psychiatric Nurse Practitioner (PMHNP) to provide psychiatric care to patients in various settings, including outpatient clinics, home visits, and potentially assisted living or inpatient environments across Brooklyn, NY. The Psychiatric Nurse Practitioner will assess, diagnose, and treat individuals with mental health disorders, offering medication management, psychotherapy, and collaborative care to ensure optimal outcomes. The PMHNP will work within an interdisciplinary team to deliver patient-centered mental health services that meet the unique needs of individuals in different care settings.
Key Responsibilities:
•Psychiatric Evaluation and Assessment:
•Conduct comprehensive psychiatric evaluations and assessments for patients in various settings, including outpatient clinics, homes, or assisted living facilities.
•Gather and evaluate relevant history, symptoms, and behaviors to assess mental health conditions and determine appropriate treatment plans.
•Diagnose mental health disorders using DSM-V criteria and develop personalized treatment strategies.
•Medication Management:
•Prescribe, manage, and monitor psychiatric medications for patients, ensuring that treatments are appropriate and effective.
•Regularly assess the effectiveness and side effects of medications, adjusting dosages and regimens as necessary.
•Provide education to patients and families regarding prescribed medications, potential side effects, and medication adherence.
•Psychotherapy and Counseling:
•Provide individual and group psychotherapy, using evidence-based techniques such as Cognitive Behavioral Therapy (CBT), Solution-Focused Therapy, or other appropriate modalities.
•Offer ongoing counseling to patients, helping them manage symptoms and improve their mental health and coping strategies.
•Deliver psychoeducation to help patients and families better understand mental health conditions and treatment approaches.
•Care Coordination and Collaboration:
•Collaborate with interdisciplinary teams, including psychiatrists, primary care providers, social workers, and other healthcare professionals, to coordinate comprehensive care.
•Participate in case management activities, including regular meetings and communication with community services, family members, and other providers involved in patient care.
•Ensure that patients have access to necessary resources, including referrals to other healthcare providers, support services, or specialized care when needed.
•Documentation and Compliance:
•Maintain accurate and detailed documentation of patient evaluations, treatment plans, progress notes, and other relevant information in accordance with state, federal, and organizational standards.
•Ensure all documentation complies with HIPAA regulations, organizational policies, and relevant clinical guidelines.
•Complete all required forms, reports, and documentation for insurance purposes, billing, and legal compliance.
•Crisis Intervention:
•Provide crisis intervention and management, particularly in emergency situations, ensuring the safety and well-being of patients in acute distress.
•Implement appropriate measures for patients experiencing psychiatric emergencies, including suicide risk, violent behavior, or severe psychiatric symptoms.
•Facilitate hospital admissions or referrals for patients requiring inpatient care.
•Patient Education and Advocacy:
•Educate patients and their families about mental health conditions, treatment options, and recovery strategies.
•Act as an advocate for patients, ensuring they understand their care options and have a voice in their treatment decisions.
•Promote mental health awareness and reduce stigma related to mental health disorders.
•Onsite Visits and Travel:
•Travel to various settings across Brooklyn, including patient homes, outpatient clinics, and assisted living facilities, to deliver care.
•Maintain a flexible schedule to accommodate patients' needs and various care settings.
Qualifications:
•Education and Licensure:
•Master's or Doctorate degree in Nursing with a specialization in Psychiatric-Mental Health Nursing (PMHNP).
•Active New York State Nurse Practitioner license with psychiatric specialization (PMHNP-BC).
•Certification by the American Nurses Credentialing Center (ANCC) or equivalent certification body is required.
•Experience:
•Minimum of 2 years of clinical experience in psychiatric care, with expertise in diagnosing and treating psychiatric conditions across a range of patient populations.
•Experience in providing care in diverse settings (e.g., outpatient, home health, assisted living) is preferred.
•Skills and Competencies:
•In-depth knowledge of psychiatric assessment, diagnosis, and treatment modalities.
•Strong communication and interpersonal skills, with the ability to build rapport with patients and families.
•Ability to work independently and manage a diverse caseload while coordinating care with other providers.
