Hood clinic, located at 24800 SE Stark Stree, Gresham, OR. Providers in this clinic include physicians and advanced practitioners. Our primary care providers are supported by individually assigned MAs as well as care teams that can include panel coordinators, social workers, RN case managers, and behavioral health clinicians and pharmacy. Our clinics support the local communities by providing services to diverse patient populations. With more than 25 clinics across the Portland metro area and rural locations such as Silverton, Keizer, Sandy and St. Helens, Legacy is helping keep the Pacific Northwest healthy.
