Cantex is seeking a dedicated Nurse Practitioner to provide holistic and compassionate care to members and their families. This role involves collaborative management with physicians and other healthcare professionals to diagnose, manage, and prevent acute and chronic illnesses, while also promoting wellness. The Nurse Practitioner will be accountable for Plan Medical Management activities, contributing to overall member health and quality improvement.

•Time split between administrative and clinical duties.
Responsibilities
•Perform in-depth health risk assessments and appropriate screening procedures for members.
•Collaborate with physicians, case managers, social workers, and other healthcare professionals.
•Review and interpret medical history, laboratory, and diagnostic data to diagnose and create comprehensive care plans.
•Prescribe and perform various treatment modalities, including medications, injections, wellness counseling, and suturing.
•Reconcile medications and recommend appropriate treatments or actions for follow-up.
•Identify unmet needs, gaps in care, and initiate appropriate referrals to PCPs, case management, and disease management programs.
•Identify overall quality improvement activities and participate in Plan Committees related to Clinical Guidelines, Quality Improvement, and Compliance.
•Practice in accordance with HIPAA and maintain control of all Protected Health Information (PHI).

Requirements
•Minimum 2 years of clinical experience as a Nurse Practitioner in a geriatric, adult, or family practice setting.
•Graduation from an accredited Adult/Gerontological Primary Care Nurse Practitioner program.
•Current RN and NP licensure in the state of practice.
•Board certification by AANP or ANCC in Adult, Family Practice, Geriatrics, or Adult-Gerontology Primary Care.
•Current CPR certification.
•Proven ability to perform comprehensive adult/geriatric physical exams and identify/diagnose chronic diseases in adults.
•Demonstrated ability to recognize conditions and behaviors placing seniors at risk and identify/document needs of clinically complex members.
•Ability to effectively reconcile medications.
•Geriatric certification (if not certified, must be obtained within 18 months of employment).
•Must meet company driving standards, possess a valid state driver's license, and be able to independently travel.
