The Bridge, an innovative and respected mental health agency, is seeking a full-time Psychiatric Nurse Practitioner to join its Assertive Community Treatment (ACT) team. This field-based clinician will provide comprehensive psychiatric care to adults with serious behavioral health diagnoses, substance use issues, and co-occurring medical conditions. Working within an interdisciplinary team, the practitioner will deliver person-centered services primarily in the community, focusing on individuals who have not succeeded in traditional office-based settings.

•Serving adults with serious behavioral health diagnoses, substance use issues, and co-occurring medical conditions
•Field-based clinician, delivering services primarily in the community (approximately 80% field-based)
•Integral member of an interdisciplinary team
•Working from a Harm Reduction and Trauma-Informed perspective
Responsibilities
•Conduct psychiatric assessments and evaluations
•Prescribe, monitor, and administer psychotropic medications
•Provide crisis intervention services to clients during regular business hours and consultation during off-hours
•Participate in clinical case conferences, team meetings, High Risk conferences, quality improvement, evidence-based practice implementation, and policy/procedure development
•Assess and monitor the physical health of clients
•Complete all clinical documentation as required by payers, funders, and regulatory authorities via an Electronic Health Record
•Complete all required administrative paperwork
Compensation
•USD 155,000 - 155,000 per year

Requirements
•Board-certified Psychiatric Nurse Practitioner, with appropriate experience or a strong desire to work in this setting; new graduates welcome
•Bilingual Spanish proficiency is highly desirable
