With over 400 employees in Southern Colorado, you will be joining a mighty team of support and administrative staff, clinicians, physicians, nurses, and others in our efforts to improve the health and wellbeing of our community. We are expanding and seeking a full-time Family Nurse Practitioner in Primary Care to serve our Cañon City and Pueblo locations, including care delivery in a walk-in clinic setting. What You’ll Be Doing As a provider, the full scope of primary care services which fall under primary care of adult and pediatric populations, including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance for this population. In addition, a Primary Care Family Nurse Practitioner is a member of the HealthTrac-Health Solutions Medical Services Team. In that capacity, they are expected to be familiar with the philosophy, goals, and objectives of the Medical Center and work collaboratively to achieve them. They must be cognizant of, and comply with, all center policies and procedures, as well as pertinent state and federal regulations. RESPONSIBILITIES As a Family Nurse Practitioner (FNP) with HealthTrac @ Health Solutions, you will play a central role in delivering integrated, patient-centered care and APM performance metrics. Under the supervision of the Director and Supervisor, your responsibilities include: Clinical Care Provide comprehensive primary care across the lifespan, including health assessments, physical exams, diagnostic evaluations, and management of acute and chronic conditions using evidence-based guidelines. Order, perform, and interpret diagnostic tests (e.g., blood work, imaging, EKGs, urinalysis) to support accurate diagnoses. Develop and adjust individualized treatment plans, prescribing medications, therapies, and preventive interventions as appropriate. Deliver preventive services such as wellness visits, vaccinations, screenings, and routine check-ups. Engage patients in shared decision-making and self-management support; educate on disease prevention, healthy habits, and self-care strategies. Prepare and administer prescribed medications, injections, and vaccinations. Evaluate behavioral health assessments and refer to behavioral health professionals as needed. Perform office-based procedures within scope of practice and participate in acute care situations (e.g., urgent triage). Care Integration & Collaboration Participate in daily huddles, case conferences, and care planning meetings. Coordinate referrals and care transitions across multiple providers and healthcare settings. Quality & Compliance Maintain accurate, timely electronic health records and documentation supporting medical necessity, quality metrics, and compliant billing; collaborate with billing and quality teams as needed. Deliver whole-person orientation, coordinated care, enhanced access, and continuous quality improvement. Participate in quality initiatives and use data to drive performance on APM measures (e.g., patient engagement, preventive screenings, chronic disease outcomes). Meet productivity, patient access, and panel management benchmarks aligned with organizational and payer requirements. Adhere to confidentiality standards, medical ethics, and Colorado statutes related to collaborative practice and prescriptive authority Professional Development Stay current with medical research and guidelines through continuing education and integration of evidence-based practices. Advance the education of healthcare team members to promote high-quality care. Physical Requirements: Mobility & Stamina: Ability to stand and walk for extended periods (typically 8–12 hours), frequently moving between exam rooms and workstations. Strength & Patient Assistance: Capacity to lift or move equipment and assist patients with repositioning, transferring, or supporting during examinations. Tasks may involve moderate weight (approximately 25–100 lb), sometimes requiring team assistance. Posture & Flexibility: Frequent bending, stooping, twisting, and reaching—both overhead and below waist level (e.g., retrieving supplies) and at or below shoulder level (e.g., handling instruments)—including maintaining awkward positions for several minutes. Fine Motor Skills: Ability to perform precise procedures such as blood draws, injections, suturing, and operate medical instruments/equipment or electronic systems. Sensory Acuity: Visual ability to read small print (e.g., medication labels, monitors) and assess patient conditions (e.g., skin tone, wound appearance). Clear auditory ability for patient communication, auscultation, and monitoring alarms or alerts. Communication: Clear speech, active listening, and accurate documentation for effective patient interaction and care coordination. Current D.E.A. Current Cardiopulmonary Resuscitation Certificate (CPR), ACLS or Advanced Life Support within 3-12 months of employment, if appropriate. Minimum of 5 years’ experience as an RN and 3 years’ experience as a Family Nurse Practitioner. Must be insurable under company-provided malpractice coverage. Computer proficiency Patient-Centered Service: Demonstrates kindness, compassion, and professionalism, maintaining appropriate boundaries and flexibility. Communication & Education: Effectively educates patients and advances the knowledge of healthcare team members to promote high-quality care. What we’d Like to See in you—the Nice-to-Haves DOT certification preferred (FMCSA/NRCME) Track record of working successfully as a part of a multidisciplinary team Experience in managed care environments Spanish language skills Must already be authorized to work in the US; sponsorships not available.
