Responsibilities
Participates in professional development
Prescribes, monitors, and manages psychotropic medications to adults with severe mental illness through an advanced and integrated assessment, diagnosis, and intervention in a collaborative practice agreement with a physician (psychiatrist). Prescriptive responsibilities are reviewed by the collaborative physician.
Provides supportive psychotherapy and coordinates the psychiatric care of a select client population with focus on client-centered mental health treatment toward the goal of recovery. Admits and discharges per the Level of Care and treatment plan objectives.
Participates in a multi- disciplinary team model to include the Health Care Home program which focuses on the holistic, integrated treatment of a person.
Works directly in the local community by visiting clients who reside in boarding home facilities. Manages medication, reviews the treatment goals and objectives, and coordinates care with the client and staff of the home.
