* Prescribes and review all psychiatric medications on assigned persons served
or patients
* Complete 17 unique public persons served encounters each day when working in
the Clinic

* Coordinates with other medical providers, pharmacies, laboratories, and
hospitals on behalf of CNS persons served and patients in the management of
general medical conditions that commonly occur including but not limited to
hypertension, diabetes, asthma, obesity,
overweight, tobacco use disorders, sleep apnea, heart disease, renal and
hepatic disease, HIV/AIDS, and hepatitis
* Documents the medication given, dose usage, amount, route, and location of
the injection site
* Collaborates with nurses and medical assistants to ensure completion of
routine or enhanced vital signs at each patient/person served visit according
to CNS Policy.
* Complete routine vital signs for priority persons served/patients when nurses
or medical assistants are not available to complete per vital signs work
instruction
* Completes all required documentation and reports completely, accurately, and
within designated time frame.
* Provides clinical experience in the medical management, crisis and behavior
management techniques
* Provisions of direct person served and patient services as a part of clinic
coverage and documentation of services according CNS standards
* Provides education regarding disease process, medications including side
effects and healthy lifestyle management
* Completes blood glucose testing with glucometer when necessary
* Participates in treatment and discharge planning meetings, case conferences,
medical staff meetings, and special programs meetings
* Consults with clinic management staff in response to client crisis and
emergencies
* Determines need for routine medical care and referral to other medical
providers, specialists or facilities.
* Assists in monitoring and coordinating physical and mental health status of
assigned clients
* Completes prior authorizations for pharmacy requests from third party payers
* Orders and interprets laboratory and diagnostic testing
* Participates in CNS internship/externship program with various institutions
of higher learning providing training, supervision and mentoring and
completing needed performance evaluations
* Account for scheduled work hours through billing, productivity or other
approved methods of documentation
* Completes evaluations and management visits for medication management and
psychiatric evaluations by performing a comprehensive mental health
assessment, including obtaining and accurately documenting in the electronic
health record
* Completes needed external documentation on behalf of persons served/patients
e.g. return to work, family medical leave, SS/SSI and other benefit forms
* Authorizes requested services in EHR such as state hospital applications,
etc.
* Participates regularly and document implementation of collaborative/ practice
agreement with assigned psychiatrics
* Complies with Meaningful Use (MU) and Medicare Merit Based Incentive Payment
System (MIPS) requirements including being champions and promoting the
implementation of items such as the patient portal, collection of allergies,
updated medication list, meaningful use referrals
* Participates and promote departmental and agency quality improvement
initiatives
* Active participation in improving medication adherence with increased use of
long-acting medications including Vivitrol
* Completes routine or enhanced vital signs for priority persons
served/patients when nurses, medical assistants, prescribers or clinic
coordinator is not available to complete per vital signs work instruction
* Attend and actively participate in staff development programs, meetings and
on committees as required
* Lead team in quality improvement initiatives
* Promotes quality achievement and performance improvement across the system
* Other Duties as assigned.
* Possess a Bachelor of Science in Nursing
* Masters prepared Nurse Practitioner in psychiatry
* Certified as a Nurse Practitioner
* Completion of accredited specialty masters or doctorate training program in
psychiatry
* Unrestricted License as a Nurse Practitioner in the State of Michigan
* Controlled Substance Registration from the Drug Enforcement Administration
* Possess Suboxone Drug Enforcement Administration (DEA) waiver
* Two years’ experience in direct work post master’s or doctorate degree and
experience in a psychiatric outpatient community mental health setting
preferable or five years’ experience post master’s or doctorate degree and
licensure as a Nurse Practitioner providing direct patient
care services to psychiatric patients in a general medical setting
* In lieu of this experience individuals who completed their internship at CNS
with direct observation of their clinical skills by CNS staff or individual
with 2 years’ experience as a psychiatric nurse providing direct patient care
services prior to becoming a Nurse Practitioner at CNS
* Working knowledge of medical terminology used in a mental health setting
* General knowledge of the community mental health system based on the Michigan
Mental Health Code and MDHHS contracts
* Ability to communicate effectively, professionally, and courteously
* Ability to be forward thinking and take initiative to accomplish goals and
objectives of the department
* Intermediate level experience with computers and Microsoft office products
* Ability to use discretion and judgmental when handling matters of a sensitive
or confidential nature
* Advanced time management, problem solving, customer service, interpersonal,
crisis intervention, and conflict resolution skills
* Prior Experience working in a fast-paced office environment with usual
pressures of managing multiple priorities with time constraints
* Ability to use a cell phone including answering the phone, picking up
messages, returning call in a timely manner and updating one’s outgoing
message based on scheduled time off
* Prior experience with the diagnosis, treatment and management of mental and
substance use disorder
* Proficiency in the use of an electronic health record
* Proficiency in the use of the Michigan Automated Prescription System (MAPS),
PMP AWARxE Program, Prescription Drug Monitoring Program (PDMP)
* Experience with the application of the Health Insurance Portability and
Accountability Act (HIPPA) is implemented in health care organizations
w. Ability to apply the applications of the Michigan Mental Health Code in
the provision of Community Mental Health services in Michigan
* Valid and unrestricted Driver's License
