Psychiatric Mental Health Nurse Practitioner (PMHNP)

In this role, you will be assigned to a facility or facilities in your local

community, allowing you to build meaningful connections with the individuals you

serve. You will have the opportunity to conduct in-person psychiatric

assessments, medication management, and implementing Medicare compliant gradual

dose reduction strategies for psychotropic medications. We understand the

importance of geriatric-focused training and continuous professional

development, which is why we provide accredited CEUs and ongoing case

consultation with our experienced team of clinicians. We are committed to

Clinical Care

* Conduct comprehensive psychiatric evaluations and mental health assessments

for LTC residents

* Develop, implement, and adjust individualized treatment plans

* Prescribe and manage psychotropic medications in accordance with Medicare and

LTC best practices

* Implement and document Medicare-compliant gradual dose reduction (GDR)

strategies

* Monitor and treat chronic mental health conditions including depression,

anxiety, bipolar disorder, schizophrenia, and dementia-related behavioral
