This role delivers client-centered care through medication management, individual counseling, care coordination, and patient education, while working collaboratively within an interdisciplinary team to support patient stabilization, recovery, and ongoing mental health management. Duties and responsibilities Conduct comprehensive psychiatric evaluations, including complete histories and mental status examinations for new patients. Provide client-centered individual counseling services. Perform interval histories and focused mental status examinations for returning patients. Monitor and evaluate the effectiveness of therapeutic interventions and prescribed medications. Prescribe, renew, and manage psychiatric medications; assess clinical status, adherence, side effects, and treatment response prior to refills. Collaborate and coordinate care with an integrated care team. Coordinate patient follow-up related to laboratory results, diagnostic testing, triage needs, and prescription refills. Coordinate and provide psychopharmacology training. Complete all documentation per Medi-Cal, Santa Clara County and AACI standards within expected deadlines. Provide patient education and counseling related to diagnosis, treatment options, and ongoing management of psychiatric conditions. Participate in clinical supervision, consultations, team meetings, and trainings. Maintain certification, professional training, and continued medical education. Complete other related duties and projects as assigned. QualificationsEducation & Experience: Master’s Degree or Doctorate from an accredited Nurse Practitioner program with a specialty in Psychiatry (CCNE or NLNAC). Current, unrestricted License as a Nurse Practitioner in the State of California. Board Certification from the California Board of Registered Nursing (BRN). DEA license in good standing, authorizing the prescribing of controlled substances. Current BLS certification One year or more experience in psychiatric nursing required, two or more years is preferred. Knowledge, Skills, & Abilities: Knowledge of DSM-V criteria for diagnosing behavioral health illnesses. Knowledge of trauma-informed and evidenced-based practices (i.e., CBT, DBT, ACT). Understanding of legal and ethical requirements of board of licensure. Knowledge of mandated reporting laws in California. Ability to exercise independent clinical judgment within the scope of license and the program responsibilities. Ability to implement evidenced-based techniques to address client’s symptomatology and functional impairments. Proficient writing skills. Ability to learn Electronic Health Records system. Verbal and written fluency in English required; Bilingual skills in a second language (particularly Spanish, Mandarin, Vietnamese) desired but not required. Competencies: Ability to communicate well with people of diverse cultural professional and experiential background. Ability to establish and maintain effective work relationships as part of a multi-disciplinary team. Working conditions Work is primarily conducted in a standard health center environment. Basic safety precautions and use of protective clothing or gear may be required. Working with clients who are seriously mentally ill, or high risk may increase exposure to secondary trauma.
