Requirements
Conduct thorough psychiatric assessments for residents in nursing homes and long-term care settings
Manage ongoing psychiatric care, including follow-up visits and medication oversight
Identify and treat a range of behavioral health conditions commonly seen in the elderly population
Initiate and adjust psychiatric medications based on clinical presentation and response to treatment
Develop and implement individualized care strategies tailored to each resident’s needs
Collaborate closely with interdisciplinary teams including nursing, medical providers, social services, and facility leadership
Provide guidance to facility staff on behavioral management and psychiatric care approaches
Maintain accurate, timely, and compliant clinical documentation
Communicate effectively with residents, family members, and care teams to ensure continuity of care
Contribute to overall quality and consistency of behavioral health services within assigned facilities
Qualifications:
Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
Active and unrestricted New York State Nurse Practitioner license
DEA registration or ability to obtain
Demonstrated ability to work independently with sound clinical judgment
Prior experience in geriatric psychiatry, long-term care, skilled nursing, or similar settings preferred
Experience managing dementia-related behaviors, chronic psychiatric conditions, and complex medication regimens is highly desirable
Strong interpersonal and communication skills
Comfortable practicing in nursing home and long-term care environments
