Position summary

Siyata Health is seeking a Psychiatric Mental Health Nurse Practitioner to provide expert mental health care for residents at Assisted Living Facilities. The PMHNP will evaluate, diagnose, treat, and follow patients in person at facilities and virtually as clinically appropriate. The patient mix includes seniors and other residents at Assisted Living Facilities, with an emphasis on practical, compassionate care that respects medical complexity, cognitive change, functional needs, and family or facility coordination.

Key responsibilities

• Provide psychiatric assessment, diagnosis, treatment planning, and ongoing medication management for residents in Assisted Living and related long term care environments.
• See patients at the facility and virtually, using sound clinical judgment to select the most appropriate visit format.
• Develop individualized plans of care that reflect psychiatric, medical, cognitive, and social factors affecting each resident.
• Coordinate closely with facility staff, families, onsite MA's, Technicians for TMS, and supervising physicians who are Neurologists and Psychiatrists.
• Support or participate in interventional treatment workflows, including TMS and other therapies used by the practice.
• Document encounters accurately and efficiently in Charm and other EHR's while maintaining timely follow-up and clear care communication.
• Travel among multiple facilities in the same area to ensure consistent service coverage and continuity of care.
• Help advance the mission of bringing new and better services and results for patients at Assisted Living Facilities.

Required qualifications

• Current licensure and certification required to practice as a Psychiatric Mental Health Nurse Practitioner in the applicable jurisdiction.
• Clinical experience working at Assisted Living or other long term care facilities.
• Demonstrated ability to care for seniors and other adult residents with varied psychiatric needs.
• Experience with Interventional Therapies like TMS and Spravato.
• Knowledge of EEG's and Brian Mapping.
• Empathetic approach, strong listening skills, and the ability to build trust with residents, families, and care teams.
• Excellent collaborator with the ability to work effectively across disciplines and facility settings.
• Comfort with documentation and clinical workflow in Charm and other EHR's.

Preferred qualifications

• Experience providing psychiatric services in facility-based outpatient models that combine in-person and virtual care.
• Background working with medically complex, aging, or functionally limited patient populations.
• Familiarity with coordinating care across several sites in a defined geographic area.
• Interest in service development, quality improvement, and expanding access to effective behavioral health care.

Schedule

This is an outpatient role with a relatively flexible schedule. The PMHNP will see patients at the facility and virtually, with service delivered across multiple facilities in the same area. Scheduling will require dependable coordination with facility operations, patient needs, and supervising clinicians while preserving continuity and responsiveness.
