Center for Human Development (CHD) is currently seeking a Dual Licensed Family Nurse Practitioner (FNP) / Psychiatric Mental Health Nurse Practitioner (PMHNP) as we extend our continuum of service within the community. We will work with adults, children, and families, addressing a broad range of social and emotional needs using evidence-based models of care.

The CHD Outpatient Site, located in Orange, MA, is seeking a dedicated and experienced dual licensed FNP/PMHNP to provide integrated primary care and behavioral health services to individuals across their lifespan. This role is essential to advancing whole-person care in a community-based setting, addressing both physical and mental health needs with compassion and clinical excellence.

Key Responsibilities:

•Deliver comprehensive primary care services including preventive care, chronic disease management, and acute care for patients from pediatrics to geriatrics.

•Provide psychiatric evaluations, medication management, and therapeutic interventions for individuals with mental health conditions.

•Collaborate with interdisciplinary teams including medical providers, behavioral health clinicians, case managers, and community partners.

•Participate in care coordination, quality improvement initiatives, and population health strategies.

•Maintain accurate and timely documentation in the electronic health record (EHR).

•Support crisis intervention and stabilization efforts as needed.

•Engage in community outreach and education to promote health equity and access.

Required Qualifications:

•Active and unrestricted licensure as both a Family Nurse Practitioner (FNP) and Psychiatric Mental Health Nurse Practitioner (PMHNP).

•Current DEA registration and prescriptive authority in the state.

•Minimum of 3 years clinical experience in primary care and/or behavioral health settings.

•Proficiency in using electronic health records and telehealth platforms.

•Commitment to serving underserved populations and working in a team-based care model.

Preferred Experience:

•Experience in integrated care models or Federally Qualified Health Centers (FQHCs).

•Familiarity with CMS guidelines, value-based care, and quality metrics.

•Bilingual or multilingual capabilities.

•Experience with trauma-informed care and culturally responsive practices.
