A healthcare organization is actively seeking a compassionate and dedicated NP/Psych NP to join their staff as their new Health Guide. In this role, the Health Guide (Np & PMHNP) will assess both behavioral and physical health conditions through a thorough history and physical examination
Responsibilities
•Diagnosing psychiatric disorders and mental health conditions
•Performs virtual SOAR (SSDI/SSI Outreach Access and Recovery) disability assessments
•Providing exemplary customer service to individuals with SMI and maintaining a NPS score of 90% or higher
•Commitment to reducing avoidable Psychiatric ER visits through timely evaluation and interaction with individuals
•Assessing behavioral health and medical conditions with a thorough history and virtual physical examination
•Diagnosing psychiatric disorders and mental health conditions
•Documenting patients' medical and psychological histories, mental status exams, diagnoses, treatment plans, prescriptions, and outcomes
•Educating patients and family members about mental health, medications, and treatment plans
•Prescribing and monitoring prescribed prescriptions for psychotropic medications, abiding by laws and regulations
•Managing and monitoring for metabolic syndrome and/or other comorbidities
Qualifications:
•2+ years of Clinical Care experience as a dual-certified Family Nurse Practitioner/Psychiatric Mental Health Nurse Practitioner and/or Adult Health Nurse Practitioner/Psychiatric Mental Health Practitioner
•1+ years of telehealth experience
•Master's Degree
•Licensed to practice Medicine in the State of New York
•Dual certification as a Family Nurse Practitioner/Psychiatric Mental Health Nurse Practitioner or Adult Health Nurse Practitioner/Psychiatric Mental Health Nurse Practitioner
•Strong background in the diagnosis of Psychiatric Disorders and Psychopharmacological Treatments
•Familiarity with non-Pharmacological interventions
•Excellent verbal and written communication, organizational, and time management skills
