Sunbelt Staffing is hiring a hybrid Psychiatric Mental Health Nurse Practitioner (PMHNP) in Buffalo, NY , to deliver integrated behavioral health services to high-risk, underserved patients. This role combines in-home psychiatric assessments and telehealth consultations , providing patient-centered care and supporting population health initiatives within a value-based care framework.
Youll collaborate with a multidisciplinary team to improve mental health outcomes across the lifespan, with no weekendsor holiday shifts required (with potential for on-call backup nurse line support in licensed states).
Patient Population & Role Overview
The PMHNP will serve pediatric through geriatric patients , conducting comprehensive psychiatric evaluations and follow-ups through in-home visits and virtual telehealth appointments. Visits may include:

Youll address conditions such as depression, anxiety, bipolar disorder, schizophrenia, PTSD, and substance use disorders. Care is delivered in accordance with Medicare, Medicaid , and commercial plan guidelines.
This role emphasizes home-based behavioral health as part of a holistic, value-based care model that reduces barriers to access, lowers healthcare costs, and drives better outcomes for vulnerable populations.
Key Responsibilities
•Conduct psychiatric evaluations via in-home and telehealth formats
•Develop and manage individualized mental health care plans
•Prescribe and manage psychiatric medications
•Provide patient and caregiver education
•Collaborate with primary care, care coordinators, and social services
•Support crisis intervention and stabilization when needed
Work Hours
•MondayFriday, 8 AM 5 PM
•No weekends or holidays (potential on-call nurse line backup in licensed states)
Assignment Details
•13?week contract to start
•4?week extensions
•Temp-to-perm potential
Required Qualifications
•Minimum 1 year PMHNP clinical experience (preferably outpatient or community mental health)
•Current, unrestricted New York NP license with PMHNP certification
•Graduate of an accredited Psychiatric Nurse Practitioner program
•ANCC board certification as a Psychiatric-Mental Health Nurse Practitioner (Across the Lifespan)
•DEA registration with ability to prescribe controlled substances in NY
•Valid National Provider Identifier (NPI)
•Reliable transportation, valid drivers license, and insurance for community-based visits
•AHA Basic Life Support (BLS) certification
•Ability to work independently in both home and telehealth settings
Preferred Qualifications
•Experience managing serious mental illness (SMI), substance use disorders (SUD), or co-occurring conditions
•Familiarity with HCC documentation, ICD-10 coding, and Health Risk Assessments
•Experience with Medent or similar EHR
•Case management or care coordination background
•Bilingual proficiency in applicable areas
