Summary The Advanced Practice Nurse (APN) Psychiatric Mental Health Nurse Practitioner (PMHNP) assigned as PCMH-I Program Manager serves as the administrative leader of a multidisciplinary team including nurses - psychologists - psychiatrists - doctoral level pharmacists - and Nurse Practitioners and also has primary responsibility for the operation and management of key clinical - training - and administrative programs within the Primary Care setting.
Duties
Duties for this position include - but are not limited to: Independently practices as a psychiatric mental health nurse practitioner providing a full range of psychiatric mental health services utilizing treatment guidelines in the delivery of high-quality care
Collaborates with patients - families - and health care practitioners to develop - implement - and evaluate care
Utilizes customer service principles - supports veteran self-determination - and serves as a resource for patients and staff in addressing ethical issues
Demonstrates critical thinking in assessing and managing complex situations
Independently practices as a psychiatric mental health nurse practitioner - utilizing evidenced-based practice guidelines in the delivery of high-quality care
Collaborates with patients - families - and the health care team to develop and implement treatment plans and evaluate health outcomes
Leads the delivery of care to assure continuity of care and peer accountability for practice - improving outcomes of panel/program
Provides leadership in the application of the nursing process to client care - organizational processes and/or systems - improving outcomes at the program or service level
