Case Management
Develop and oversee implementation of patient care plans
Participate in monthly Quality Improvement meetings with the Medical Director
Assist all staff in reaching clinical measures and quality goals
Team Collaboration
Active participation in weekly team meetings and monthly team plannings, providing insight into programmatic design and initiatives
Precept and supervise volunteers and university students, as needed
Work collaboratively with the clinic team, other medical providers, mental health therapists, and registered nurses to provide integrated healthcare to patients
Collaboration with the Clinical Director and Outreach Director to provide ongoing clinical support and outreach medical services
Required Qualifications
Licensed/certified Nurse Practitioner or Physician Assistant in the state of North Carolina
Advanced English and Spanish fluency with excellent written and oral communication skills
Willing to participate in outreach events in surrounding counties, as needed
Cultural sensitivity, a caring and confident demeanor, flexibility, dependability, professional attitude and desire to serve uninsured/underserved populations
Preferred Qualifications
Minimum 2 years’ experience as an advanced practitioner
At least 2 years’ experience in a multidisciplinary care team setting
Native or near-native fluency
Working knowledge of farmworker-specific medicine
