Job Summary

We are seeking warm and razor-smart Psychiatric Nurse Practitioners with entrepreneurial spirits to provide psychiatric consultations in nursing homes. This is a travel-based role that combines in-person visits at assigned nursing facilities with telehealth services. Candidates must be comfortable traveling throughout Vermont to provide in-person patient care while also conducting consultations and follow-ups via telehealth.

As a Psychiatric Nurse Practitioner, you will be provided with full administrative support and clinical mentorship. Responsibilities

  • Travel to assigned nursing homes throughout Vermont to provide in-person psychiatric consultations and patient care.
  • Conduct psychiatric consultations and follow-up appointments through both in-person visits and telehealth.
  • Work closely and collaboratively with nursing home staff, residents, families, and other members of the care team.
  • Proactively follow up with patients and assess the efficacy of recommended interventions.
  • Provide education to patients, families, and facility staff on psychiatric best practices.
  • Round at assigned nursing homes and provide psychiatric medication and treatment recommendations.
  • Integrate pharmacological and behavioral interventions, emphasizing non-pharmacological treatments whenever possible.
  • Maintain timely, accurate, and thorough clinical documentation.

Requirements

  • Master of Science in Nursing (MSN)
  • Active Psychiatric Nurse Practitioner License
  • ANNC Board Certification
  • Ability and willingness to travel throughout Minnesota for in-person nursing home visits
  • Comfortable providing care through both in-person and telehealth modalities
  • Excellent communication and documentation abilities
  • Strong patient assessment skills
  • Strong, ambitious, patient-centric work ethic
  • Prolonged sitting. Computer work including keyboard and mouse use for extended periods. Lifting/moving patients.
